    About

    Tombolo’s Mission

Tombolo’s mission is to help families of children with ongoing health conditions in the Twin Cities metro area by connecting them to high quality community resources, mobilizing their existing support networks and providing volunteers to address their unmet needs.

    Tombolo’s Vision

Tombolo’s vision is strong, connected and engaged communities that support individuals and families with dignity and respect.

    Why are we called Tombolo?

tombolo is a ridge of beach material (typically sand), built by wave action, that connects an island to the mainland.  We too seek to harness action, in this case human caring, to connect families to a larger body of support.

Why is Tombolo important to our community and families?

In the 21st century, “there has been an increase in the number of Americans living with one or more chronic conditions. And unlike most infectious diseases or sudden acute illnesses, chronic conditions, by our definition, last a year or longer and limit what one can do and/or require ongoing medical care. In this new century, we have to confront a new reality: growing numbers of people with chronic conditions seeking health care in a system that is not structured to respond to this new dynamic (Anderson & Horvath, 2002).”
